This Notice describes how medical information about you may be used and disclosed, and how you can access this information. Please review it carefully.

Your Rights

At ResilienceADHD, we’re committed to providing the highest level of security for our patient records. As per HIPAA, all treatment sessions and test results are highly confidential. No records or results are released to any third party without prior written consent from the patient.

 

You have the right to:

  • Get a copy of your medical record (paper or electronic).
  • Request corrections to your health information.
  • Request confidential communications (e.g., by phone, by mail, or email).
  • Ask us to restrict certain uses/disclosures of your health information.
  • Receive an accounting of disclosures (list of when your health info was shared).
  • Receive a copy of this Notice at any time.
  • File a complaint with us or the U.S. Department of Health & Human Services if you believe your privacy rights have been violated.

Our Uses & Disclosures

We may use and share your information for:

  • Treatment: Sharing with clinicians involved in your care.
  • Payment: Billing your insurance or collecting payment.
  • Healthcare Operations: Quality improvement, staff training, accreditation.
  • Public Health & Safety: Reporting abuse, preventing serious threats, complying with legal orders.

Our Responsibilities

  • We are required by law to maintain the privacy of your Protected Health Information (PHI).
  • We must notify you in the event of a breach of unsecured PHI.
